Most first visits to our office consist of an oral exam and a consultation, where your diagnosis and options for treatment will be discussed. On occasion, procedures that are simpler and more straightforward may be performed on the same day as the consultation.
Please bring the following information with you to your first visit:
- Your referral slip from your dentist
- X-rays, if applicable
- List of current medications
- Medical and/or insurance information, including necessary completed paperwork and insurance card(s)
- Driver’s license or other government-issued ID
If you are under 18 years of age, you must bring a parent or guardian with you for every visit to our office, including your first visit.
If you have diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, or any other medical condition that may be of concern prior to surgery, please let us know. Alerting us to these and other conditions is important for your safety.
